UHD's Hospice program is centered on a philosophy that accepts death as
the final stage of life. The goal of our program is to enable patients to continue an alert, pain-free life and to manage other symptoms
so that their last days may be spent with dignity and quality, surrounded by their loved ones.
Hospice care treats the person rather than the disease; it focuses on quality of life rather than length of life. Hospice care is
family-centered care—it involves the patient and the family in making decisions. Hospice care can be given in the patient's home,
a hospital, nursing home, or private hospice facility.
Our program is comprehensive team-based care of a medical director, hospice director, skilled nurses, home health aides, social workers, a massage therapist, chaplain and volunteers. There is a nurse on-call 24-hours per day, 7 days per week.

UHD's Home Health Care program provides professional services, support and individualized health care in the home setting. A registered nurse plans and supervises the Home Care program and reports progress to your physician. Our professional staff continues care at home for as long as needed or until independence is regained.
