Transitional care is a service provided by United Hospital District which meets the special needs of recently discharged hospital patients, out patients and patients needing short-term nursing care. The service was developed to accommodate the needs of discharged patients who wish to remain within the hospital environment for a period of time before returning home.
To help ensure a quality transition from one environment to another, UHD is pleased to offer the Transitional care Program. The cornerstone of this program is its emphasis on independence, self-care and the opportunity to nurture skills in preparation for returning to familiar surroundings.
Patients on Transitional Care stay in our private rooms with telephone, bathing facilities and a nurse call system. The services you will receive include:
Nursing care appropriate to your needs
Linen service
Three meals a day
Housekeeping service
Use of lounge for social activities
Mail delivery
