Immunizations & Well-Child Visits

Students from kindergarten through college may need updated immunizations based on school requirements. Our providers can help determine what your child needs based on age and guidelines.

Sports Physicals

Students participating in school sports need an updated physical every three years. Only $30 when paid at time of service. If billed through insurance, sports physicals will process as a Well-Child Visit.

FACE MASKS

Updated October, 9, 2025

Please note that medical-grade face masks are required for patients with the following symptoms at UHD Hospital and Clinics: Patients and visitors experiencing fever, chills, muscle aches, runny nose, nasal congestion, cough, sore throat, headache, vomiting, diarrhea, and skin rash symptoms are asked to wear a medical grade face mask. UHD will provide this upon entry, if needed.

UHD remains committed to monitoring respiratory illnesses and will continue to follow guidance closely and update policies as necessary. Thank you for helping UHD continue to provide a safe environment for our patients, visitors and staff.

  • Monday, May 25th: Memorial Day
  • Friday, July 3rd
  • Monday, September 7th:  Labor Day

  • Friday, December 25th: Christmas Day
